In accordance with the Archdiocese, all athlete's are required to have the following forms on record with the school. Please print off the appropriate forms for your student's sport, fill it out and return the forms to the Athletic Secretary through the School Office. All forms are formatted in Adobe Reader. If you don't currently have Adobe Reader you may download from the link below.
All forms and fees will be required before any athlete will be permitted to practice or receive a uniform.
One copy per year of the following forms need to be on record for all athlete's.
